PAYMENT POLICY

Medical services are provided and billed by Restoration PT. Our providers participate in most all of the major health insurance plans in the region. Following are the health plans which we currently accept.

If your plan is not listed, just give us a call. We can verify your network and benefits and we may be able to file claims on your behalf. However, we can also provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.

Please be aware that with any health plan, there may be deductibles and copayments for which you could be responsible.

FREQUENTLY ASKED QUESTIONS

Can I choose where I want to have Therapy?

  • Yes, you may choose where to have your physical therapy. Your physician may have their own preference, but you have the freedom to choose. Majority of our patient choose us because of or dedicated 20-30 minutes of one-on-one hands-on manual therapy based on your needs upon assessment.

Do I need a physician referral to receive therapy?

​

  • A physician referral is required by health insurance for you to see one of our licensed physical therapists. In special cases, an initial evaluation may be performed for assessment but subsequent visits and treatment interventions will require an official referral by the physician. What can I expect during my first visit?

​

  • Upon signing our consent to treat form, the therapist will perform a thorough assessment based on your history and clinical presentation. The first sessions are approximately 60 mins in length consisting of mostly evaluation followed by treatment through initiation of a personalized home exercise program. Your next visit will be dedicated fully toward treatment and reassessment of your progress toward your goals.

  • Please try to arrive ~10-15 minutes before your appointment if you would like to fill out our forms in person.

What do I bring to my first visit?

​

Please bring:

  • your "new patient" paperwork and the appropriate survey form (ie. shoulder, ankle, back, etc), located on our forms tab

  • your personal ID (Driver's License, passport, etc)

  • Insurance card(s)

  • Physician's Referral

  • If you have it, please bring a copy of appropriate imaging results you have either CD or radiologist report

What do I wear?

  • If possible, please wear comfortable clothing (gym shorts, sweatpants, t-shirts, etc) to allow us to access the appropriate body part for assessment. For higher level athletes, appropriate athletic shoes may be needed to assess your gait and running mechanics. You are welcome to bring clothes to change into before and after your session, if needed.

How long is each visit?

  • Each session can range from 30-60 mins

  • First 20-30 mins are dedicated one-on-one treatment with the physical therapist (usually manual therapy) followed by modalities and therapeutic exercises under physical therapist and technician supervision

How often do I go to treatment?

  • Frequency of treatment is determined during your initial evaluation. Patients are typically seen 2-3x/week and progressively decreased until the patient is able to independently manage their symptoms.

QUESTIONS ABOUT YOUR BILL

What billing or insurance information will I receive?

  • ​You will receive a copy of your bill. After your insurance has paid, we'll provide you with a monthly statement of your account.

​

How long will it take to get things settled with the insurance company?

  • Generally, it takes from 10 to 25 days to obtain payment from an insurance carrier.​

​

​Why am I getting bills from physicians?

  • ​Each physician who provides professional services will send you a separate bill from his or her office. These physicians will include:

- your own physician
- other physicians who conducted tests and procedures
- consulting physicians chosen by your primary physician to read and interpret test results.

These physicians may include specialists in anesthesiology, cardiology, emergency medicine, nuclear medicine, pathology, radiology, or urodynamics. You will be responsible for paying these bills as well.

​I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?

  • ​If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
